First and foremost, make sure that any new concrete is installed with a slope leading away from the home. In other words, during rainfall or while sprinklers are turned on, water should easily drain away from the home and not pool or puddle near the foundation. If you notice any areas where water is getting trapped or pooling near a patio or a walkway, you can add a drainage system such as a trench or a French drain to assist the water flow away from your foundation. 

If you are adding a walkway, driveway, or patio that will lead directly to your home, it's important not to tie it directly to your foundation. Leave a gap or install an expansion to allow the new concrete to move independently without causing stress to the foundation. This will ensure longevity and durability for your new flatwork installation as well as protect the structural integrity of your home.
